Aetna Care Mana ger $1,000 Signing Bonus! Status: Full-time (37.5 hours/week)
Pay Range: $55,000 - $70,000 (paid hourly)
Work Location: Remote except for when conducting client visits or participating in required in-person meetings or training sessions.
Job Summary: Care Managers in Council on Aging's Aetna program make a difference every day in the lives of older adults by working to help them remain in their homes - where they want to live. Aetna Care Managers work with clients, caregivers, health care professionals, service providers and an integrated team to ensure the client receives the right services in the right amount to accomplish this.
Essential Functions: - Providing support/education to clients with complicated social/behavioral/environmental issues
- Conducting home visits
- Utilizing clinical judgment and critical thinking skills to facilitate appropriate client physical and behavioral healthcare and coordinating psychosocial and medical wraparound services to promote effective utilization of available resources
- Reporting specific health and social information back to the integrated team to assist in development of care/service plan
- Advanced care planning with client and others involved with client care
- Authorizing services in the most efficient means possible to meet identified client needs
Required Education, Licensing & Certifications: - Ohio Licensed Social Worker (LSW) or Registered Nurse (RN)
Experience: - At least one year of experience working with people with disabilities/chronic conditions and long-term services and supports.
- Knowledge of the individual, family, social and the individuals functioning in the aging process.
- Prior experience completing electronic documentation
- Prior case management experience and experience working in a program with strict and multiple contractual requirements is a plus
Skills & Abilities: - Critical thinking skills
- Ability to effectively use clinical judgment
- Ability to learn in-person, through a written format, and virtually
- Have above-average technology skills
- Ability to work in multiple software systems simultaneously
- A good working knowledge and understanding of issues related to the older adult population.
- Knowledge of community resources within southwestern Ohio
- Good oral and written communication skills
- A valid driver's license and reliable transportation for travel and conducting home visits and attending training sessions
- Proficient computer skills including Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed
